Register as a New Patient

We welcome new patients to our surgery. If you would like to see one of our doctors, please call us in advance on 01 805 8508 or drop into the surgery and our team will be delighted to show you around and chat to you about the practice and our services.

When you arrive at the surgery for the first time, you will be asked to fill out a patient registration form which we would ask you to complete as comprehensively as possible. You will then meet with one of our doctors. Any paper work in relation to obtaining your previous medical files will be filled in with the doctor and any forms required to change over a GMS card will be completed at this time.
